Medicare Announcement

Sandhills District Health Department and Medical Clinic (SHDHC) wants to make you aware of a new service available to Medicare recipients. Medicare now covers an Annual Wellness Visit at no cost to the patient. It is designed for people who are enrolled in Medicare for one year or longer and have NOT had a physical in the past 12 months.

The visit includes:

     History
     Evaluation of cognitive and functional ability
     Depression screening
     Evaluation of Physical Status
     Evaluation of Health Promotion practices
     Identification of disease risk factors and interventions that may be indicated
