Kids vaccine available on walk-in Wednesdays or by appointment. More info at cdc.gov and flu.gov

Children 6 months to adults of any age should have their annual flu vaccine. Children under 9 that have not had 2 flu shots in one season will need 2 vaccines between now and April/May. Those who have had 2 flu shots in one flu season will only need one flu shot this fall to be fully immunized. Children who have hit their 10th birthday and above only need one vaccination.

Instructions: Fill out the name/address section and sign the form and bring with you.  SDHDC staff will fill out the insurance section and the vaccine section.
